Two cleanings per year
Two preventative clinical fluoride treatment applications
Emergency exams and radiographs as needed
Routine x-rays with exam
Routine referrals to specialists as needed
15% off all restorative, surgical, and cosmetic treatment
No annual maximums, waiting periods, or deductibles

- Benefits run for one full year from the date the membership form is signed, must be used within that period, and are non-transferable to any other party. Unused services expire at the end of the term and cannot be rolled over.
- This membership is not an insurance plan and cannot be combined with other insurance or any other discount plan.
- Membership is non-refundable.

- Discounts apply only to services offered within this office, are at the doctor's discretion, and do not extend to any procedure referred to a specialist.
- A new plan is required for a patient once they have been a member for one year.
